New Patient Registration

The practice is happy to register anyone who lives in the G41, G51, G52 postcode areas.

All new patients will be required to complete a registration form and a medical questionnaire prior to being registered. We also require information regarding eligibility to register as a NHS patient.

The facility to register remotely by email has been created. An editable PDF file can be dowloaded, filled in on your computer, tablet or phone and emailed back to the surgery at GGC.GP52217clinical@nhs.scot. The form can be downloaded from the link below

We also ask that you complete and return the medical past history form, which can be downloaded from then link below.

New patient medical history form

Please use the document below if you are registering a child under the age of 16.

New (under 16) patient medical history form

Please provide two forms of identification - for adults this must include one form of photographic ID (passport, driving licence) and proof of current address such as a utility bill dated within the last three months. It is strongly preferred that this evidence is provided, but you can still register if it is not available to you.

  • Please complete both forms as fully as possible
  • Return the forms to the practice before 12 noon on any working day. We cannot process forms in the afternoon.
  • Registration will be processed within 48 hours in the practice and can take 7 days for the Health Board to complete the process.
  • Please bring photo ID and proof of address when returning the forms (if available).

New patients are invited to attend for a free health check with the practice nurse. Details are available from reception.
